How do I request a Subject Access Request

As an individual, you have the right to access and receive a copy of your personal data. This is referred to as a Subject Access Request or 'SAR'.

Here is what you should include when requesting a Subject Access Request (SAR):

  1. Subject Line: Use "Subject Access Request" as the header.
  2. Date: Include the date of your request.
  3. Your Information: Provide your name, previous names if applicable, email address, home address, and phone number.
  4. Identification Numbers: Include any relevant identification numbers, like customer account numbers or NHS numbers.
  5. Requested Information: Specify the personal information you're seeking and what you don't need, if applicable.
  6. Details to Help Locate Information: Provide any relevant dates or details to help the organization find the information.
  7. Reason for Request: Optionally, you can explain why you need the information.
  8. Preference for Receiving Information: State how you'd like to receive the information (electronically or by post) and any accessibility requirements.

Submitting a clear and concise SAR helps ensure that you receive the information you're seeking efficiently. Once we have received your request, we must reply to the request within 30 calendar days, unless there are any legal requirements/restrictions. A fee may be charged in certain situations.
